Affidavits & Sworn Statements in DFW

Jurats and oaths for affidavits, declarations and sworn statements.

What this service covers

An affidavit is a written statement you swear or affirm is true. Notable Notary administers the oath, watches you sign and completes the jurat, so the statement can be used by a court, an agency, an employer or an insurer.

I cannot draft or advise on the content of an affidavit. An attorney or the requesting office can help with wording.

  • Affidavits of identity, residence and heirship forms prepared by your attorney
  • Small estate and support affidavits prepared for you
  • Sworn statements for insurers and employers
  • Declarations and verifications for agencies
  • Affidavits of support and gift letters

What to bring

A few minutes of preparation keeps the appointment short. Here is what every signing needs.

  • The completed affidavit, unsigned
  • A current government-issued photo ID
  • Any exhibits or attachments referenced in the statement
Brass magnifying glass over a photo ID during notary identity verification

Accepted ID usually includes

  • Texas driver license or ID card
  • U.S. passport or passport card
  • Driver license from another state
  • U.S. military ID
  • Foreign passport

How it works

  1. Read it through

Make sure the statement is complete and true before the appointment. I cannot fill in or change content.

  2. Take the oath

I ask you to swear or affirm that the contents are true. You choose oath or affirmation.

  3. Sign in front of me

You sign the affidavit in my presence, which is what the jurat certifies.

  4. Jurat and seal

I complete the jurat, sign, seal and record the act in my journal.

Common questions

All questions

Do I have to swear on anything?

No. You can take an oath or make an affirmation, which carries the same legal weight without a religious reference.

Can you notarize an affidavit that is not in English?

I need to be able to communicate directly with you and to read the notarial certificate. The certificate must be in English; ask before booking if your document is in another language.
